Public Health Nurse

Full-time, Mon- Fri

Performs registered nursing duties as established by policies and procedures set forth by the USDA/Women, Infants and Children (WIC) program, the Family Case Management (FCM) program, the Adverse Pregnancy Outcomes Reporting System (APORS) Program, Genetics, Immunizations, the Lead Program, Communicable Disease Program, and the State of Illinois.

REQUIREMENTS:

  • Active Illinois Registered Nurse license,
  • Proficient computer skills
  • Preferred: current CPR certification, phlebotomy skills

 Family Resource Advocate

Full-time, Mon- Fri

Schedule is flexible and non-traditional; based around program event need and families served.

Provides case management and family advocacy through the Effingham County Family Resource Center.

REQUIREMENTS:

  • Associate’s Degree required. Bachelor’s Degree preferred.
  • Minimum 2 years’ experience working in social services, family case management, child welfare or a similar field.
  • Experience working with at risk populations and child welfare preferred.
  • Must be able to work independently in a startup environment.
  • Detail oriented and proficient Microsoft applications.
  • Driver’s license and reliable transportation required.
  • Oral and written fluency required.

 Medical Case Manager Assistant – HealthWorks of Illinois

Full-time, Mon- Fri

Performs assigned duties related to assessments, client demographics, medical/health history and exams, and assists clients’ and substitute caregivers’ transition through the Department of Children and Family Services System.  Executes skills mandatory for the Statewide Automated Information System (SACWIS) and Illinois Department of Public Health Cornerstone System.  Communicates daily with various social service agencies and healthcare providers.

REQUIREMENTS:

  • Paraprofessionals and lay workers may perform some case management under the supervision of the HealthWorks Coordinator or a Case Manager. (ftp://www.ilga.gov/JCAR/AdminCode/077/077006300D02200R.html)
  • 3+ years’ experience in social and/or family services-based program preferred.
  • Detail oriented and proficient in Microsoft Word and Excel.
  • Computer skills and tech savvy required.
  • Able to demonstrate problem-solving skills.
  • Independent, self-motivated worker.
  • Must be able to sit, stand, and walk 70-80% of the time.
  • Must be able to stoop/kneel to file and retrieve files.

 Emergency Preparedness Coordinator

Full-time, Mon-Fri

Emergency Preparedness: Build and sustain the Health Department’s public health preparedness capacity. Ensure compliance pertaining to deliverables of the Public Health Emergency Preparedness (PHEP) grant. Prioritize a risk-based approach to all-hazards planning to improve readiness, response, and recovery capacity for existing and emerging public health threats. Support whole community readiness, response, and recovery through partnerships and communication systems for timely situational awareness and risk communication. Work closely with local, regional, and state partners. Maintain the Effingham County Medical Reserve Corps (MRC) unit.

Environmental Health: Perform office and field work, including educational and enforcement activities, related to the regulation of food establishments. Conduct activities related to vector surveillance. Provide support for other Environmental Health programs as requested.

REQUIREMENTS:

  • Associate’s Degree required, Bachelor’s Degree preferred
  • IL Driver’s License and appropriate reliable transportation required.
